## Narrative

Employee notified mgmt of ongoing pain in shoulder that wasn't going away. EE has had pain historically that was treatable with ibuprofen. Employee placed on restrictions awaiting surgery for shoulder repair. MRI reveals torn subscapularis tendon, supraspinatus and infraspinatus tendinopathy, AC joint degeneration and labral fraying. MRI results received on 5/18/16.
