Medical Evidence received on May 27, 2016, indicate that this ee was granted a 10% permanent partial disability award for Occupational Pneumoconiosis. The accident occurred because the ee failed to use respiratory protection. EE was properly trained, did not violate any rules, regulations or policies, has a good attitude but is a multiple injury ee. Claim 10/13/15.
