Narrative

The person was changing a coupler on a 8"" Aqua Mine pipe. EE was standing on a 6' ladder. When EE stepped off the ladder, EE stepped on the rung on the side instead of the middle causing the ladder to tilt. EE fell on back on the right side. Resulting in 3 fractured ribs.
