## Narrative

While working with a long wall duster, the employee's pants leg caught and uncoupled a pressurized hose, striking EE in the left knee. The accident was caused by the hose clamp not being secured by the employee when assembled.  The employee did not violate any rules, regulations or policies.  The employee has a good attitude and was properly trained on the task.
