Narrative

Employee was installing a 6' roof fully grouted roof bolt which had to be bent before installing. When straightening the roof bolt it spun around causing EE to fall forward. This caused a sharp pain in EE's back. EE was taken for medical treatment and was unable to return to work next shift.
