Narrative

Employee picked up sample from the back of a pick-up truck, extended left arm to sit the sample on the dock at which point EE's shoulder ""popped"". At this time pain is not such that employee wishes to seek medical attention. Over time employee stated EE is beginning to have issues with shoulder. EE is now on restricted duty as a result of Dr. visit on 6/8/16
