Narrative

The mine's Safety Department was notified on 6/27/2016 that this retired employee has been granted an award of 2.2% for noise induced hearing loss. Date of last exposure was 03/23/2015 and date of filing was 12/09/2015. This incident was caused by the employee's failure to use hearing protection during career. EE did not violate any rules, regulations or policies.
