## Narrative

Employee notified the company on 7/22/13 about worsening pain in shoulder. Employee came back to management in April 2016 requesting to be seen by a doctor for the shoulder since it was becoming a worsening problem. Employee missed no days until 6/20/16 when EE had surgery for right shoulder. No known event occurred.
