The employee was attempting to straighten an 8' roof bolt & install it in the roof. When they tried to bend the bolt, they lost their grip & the bolt spun causing them to fall & injure shoulder. The following were not a factor: Rules, Regs, Mining Equipment or Systems, Skills, Proficiency, Training & Protective Devices on Equipment or Clothing. Attitude may have been a factor.
