Narrative

Was pulling a charge pump onto landing when I slipped & holding onto the pump so I wouldn't fall I strained my back. Cause of injury was wet conditions. Incident didn't involve any aspect of compliance with rules/regulations/PPE. Miner proficiency may have been a factor. Started missing work 6/30/16.
