## Narrative

Employee tripped and fell while measuring a silo. When EE lifted the cover, the silo was  blowing dust out. EE turned head quickly to get out of the dust and tripped. EE fell forward onto the silo lid that is a 3'X3' box 18"" off of the ground. EE's shoulder hit the lid first then the right side of face.
