## Narrative

 Was down in the #1 reclaim tunnel on the #4 feeder landing changing a nozzle on hose to a wand. Feeder broke free causing surge of material to pin against hand rail of the platform. Cause of injury material falling due to wet conditions. Incident didn't involve any aspect of compliance with regulations/rules/PPE. Miner proficiency may have been a factor. Started missing 7/8/16
