Narrative

Was washing out #3 feeder in #2 reclaim when a large chunk broke free & wedged between feeder belt & catwalk. Catwalk raised & lost balance falling from platform. Cause of injury material falling due to wet conditions. Incident didn't involve any aspect of compliance with regulations/rules/PPE. Miner proficiency may have been a factor. Started missing 7/18/16.
