Narrative

EE was traveling back outside from a remote area of the mine. In location inby Spad #1551 a roof rock 12x16x3 fell striking EE on left side of face & shoulder. Impact of rock caused a 1"" cut on side of face did not require stitches. EE lost consciousness for a period & regained consciousness and called for assistance.
