Narrative

Employee was operating the #419 roof bolting machine on the 1 North Section. EE was bolting in the #5 left entry. In the process of EE starting to drill a hole in the top to install a roof bolt, a piece of rock that measured 26""X10""X8"" fell from the mine roof and struck the injured employee in the right hand.
