## Narrative

While installing a roof bolt, the bolt coupler caught the inner edge of the roof strap causing the bolt to bend striking the employee's right index and middle finger.  The accident occurred because the ee failed to release hand off the up pressure.  The ee had on appropriate PPE, was properly trained and has a good safety attitude.
