An roof fall occurred on 8-3-16 on old mainline at 4 North Track at survey station 213+00. Fall was about 8' above the anchorage point and 11 by 15' wide at the intersection to 4 north.The cause of the fall was due to weathering of roof.Supplemental bolting is being done in these areas.There were no compliance issues and no other part 50 criteria applies
