Narrative

Employee was attempting to remove a piece of belting from underneath a belt tail roller with a set of come-a-longs. As EE tightened the come-a-long, the belt dog slipped. The sudden release of tension caused right arm to move suddenly. EE reports feeling a ""pull"" in the shoulder.
