Narrative

Loading roof straps onto Joy miner in 3 East at end of shift. Strap got caught on miner frame, causing hand to slip off and run right forearm across sharp edge of roof strap, resulting in a laceration to right forearm requiring 7 sutures. No problem with ppe, training, mining methods or compliance issues.
