## Narrative

Employee was dragging rock dust hose when EE tripped over a loop of hose.  The employee fell striking EE's head on belt structure and jammed EE's neck.  The employee worked till 08/26/2016 when EE was diagnosed with a bulging disc in EE's neck and taken off work.
