## Narrative

Employee was driving when hit some bumps in the road and felt discomfort in back.  From this, drove to Phase 1 Que Point and contacted a supervisor. Employee was transferred to the clinic for evaluation. Employee states was in 3rd gear travelling 10 MPH. On 8/22/2016, the employee returned to the clinic and was placed off work.
