Narrative

EE was positioning liners when the mechanic on the outside of the chute was assisting with a spud wrench to line up the liner to insert a bolt. The ee in the chute got finger in the bolt hole of the liner, while the employee out of the chute inserted the spud wrench to line it up. The finger was smashed in-between the liner and the spud wrench.
