## Narrative

A roof fall occurred above the anchorage point on  Main East Track at 738 crosscut. Fall was approx 25' w, 20' l and 7' h. Cause of fall was due to weathering of roof along the rib. To prevent further falls in this area we plan to add supplemental roof support and spray some areas with a strength enhancing cementitious material no other part 50 criteria apply
