## Narrative

Employee notified supervisor of pain in shoulder on 7/18 while performing normal maintenance activities. EE requested medical attention and subsequently had surgery on 8/31/16 to repair torn rotator cuff, subacromial impingement syndrome, and long head bicep tenosynovitis. EE's condition has been attributed to mine maintenance activities over the course of EE's career.
