Narrative

EE hit elbow on a guard rail 8/3/16. EE continued to work full duty without difficulties. EE was seen in the clinic on 9/12/16 for continued right elbow discomfort. On 9/14 was seen by Dr.EE was prescribed an anti-inflammatory and helio pad for the discomfort. Dr. cleared EE to continue regular duties so EE is not on any restrictions at this time.
