## Narrative

Rib bolting on right side of miner (066 MMU), retracting steel from hole while rotating to keep hole clean. When steel exited hole, excessive rotation speed caused drill steel to fly loose and struck EE in the nose, resulting in a fracture and laceration. No problems with mining methods, training, PPE, or compliance issues.
