Narrative

Profiling 15 belt at 346 block with a come-a-long attached from the structure to a belt hanger. Head coal (20""x10""x5"") fell out from around the belt hanger and hit EE in the hard hat, knocking EE down. Being evaluated for a concussion. No problems with mining machines, PPE, training, or compliance issues.
