Narrative

While pulling a leg jack from 189 shield to 190 shield, the leg jack came over the relay bar on 189 shield and pinned employees foot against the relay bar on 190 shield. This did not involve any compliance issues. No equipment involved. Personal protective equipment was being used. This did not involve job proficiency, training, or attitude.
