Narrative

Employee was removing the pillow block from the shaft on a pulley located in a cradle on the floor of Fines Repair. EE was removing bolts from the top half. EE placed EE's left hand under the block to push the bolt head back up in the pocket. EE triggered the impact wrench and caught EE's left index finger tip between bolt and pocket.
