## Narrative

On 9/26/2016 a roof fall occurred above the anchorage point. The fall occurred at survey station 38+50 and was approximately 20' long 12' high and 16'wide. The fall occurred due to deterioration of the mine roof. Supplemental support will be  installed in areas that show deterioration. Ventilation was not impeded but travel ways were. No other part 50 criteria applies.
