Narrative

Trying to put side motor cover back on hinge on #3 D7 Dozer (at the refuse impoundment) and felt pain in EE's lower back. Continued to work until 10/3/16 when EE was put off work pending evaluation from specialist. No problems with mining machines, PPE, training or compliance issues.
