## Narrative

Employee was installing a 4' supplemental bolt in uneven roof with a dual boom bolter. When employee torqued the bolt, the bolt head snapped. As employee lowered the bolter mast the 10""x24"" bearing plate fell, ricocheted off the mast and struck employee in the neck.
