## Narrative

Unplanned roof fall discovered reportable in 23W #2 entry between 2 and 3 crosscut. Fall was approx. 10' high, 20' long and 16' wide. Most likely due to presence of white chalky/clay substance in a geologic stratification layer 1-2' above anchorage which assisted in layer separation. No problems with mining machines, PPE, training, or compliance issues.
