On 9/29/16 employee reported to the company EE's right hand was bothering EE & EE was going to seek medical treatment. EE stated there was no event, impact or injury, that it had started acting up over the course of time the last few months. EE was seen 10/5/16 by a physician & diagnosed with tendonitis in the right hand & restrictions were issued. 1st shift missed was 10/7/16
