## Narrative

While employee was standing on the off-side of roof bolter #13, employee was struck in the right side of head and face by a rock that fell from the top which was 9' high. Injured went up to the bolter to ask the bolt crew if they needed any supplies when the rock fell from the top. The rock measured 42""long x 18""wide x 3"".
