## Narrative

Employee had unloaded supplies off fork truck. Went back & stepped onto fork truck, got to second step on ladder, turned & foot slipped, toe went into mud on ground & fell forward. Cause of injury wet, muddy conditions. Incident didn't involve any aspect of compliance with regulations/rules/PPE. Miner proficiency may have been a factor. Started missing work 10/24/2016.
