## Narrative

Crawler Mobile Tailpiece (CMT) cats were stuck and EE was attempting to free them with assistance of 3 ton chain hoist. The tail hook slipped off the CMT cats and flew off under tension, striking EE in the upper lip, resulting in a laceration. Received 23 sutures. No problems with mining methods, training, PPE or compliance issues.
