## Narrative

Wearing utility knife on chain hanger off belt, beside radio. Knife safety latch hit radio and extended blade, unknowingly. Miner went to pick up ventilation tube and ran left forearm across the open knife blade resulting in a laceration that required 3 sutures to close. No problems with PPE, mining methods, training or compliance issues.
