Narrative

Employee was shoveling on plant feed belt & picked up big load of mud on shovel, when turned to put on belt strained back. Cause of injury to big of load & body positioning. Incident didn't involve any aspect of compliance with regulations/rules/PPE. Miner proficiency may have been a factor. Started missing work 10/28/2016.
