Narrative

A roof fall occurred on the 10 Left section belt line on top of the section feeder. The fall was approximately 28'L x 16'W x 12'H. The area was bolted on 4'x 4' pattern with 8' Super-Twist primary bolts and two (2) 16' cable bolts spaced every 8'.
