## Narrative

Continuation of roof fall from 1/4/14 was discovered in 1st x-cut outby #2 seal of KSeals (15'x15'x9' high). Fell just above anchorage, with top 2' consisting of unconsolidated rocks with a heavy yellow residue, indicating past presence of water, causing deterioration of strata. Previous fall was over a sump. No problems with mining machines, PPE, training or compliance issues.
