## Narrative

EE was working to repair the broken conveyor belt on the #5 belt line. As they were pulling the conveyor belt a chain hook slipped causing the chain to strike a roof jack pipe. The roof jack pipe struck one victim causing that person to strike EE knocking EE into conveyor belt structure. The impact caused several broken ribs.
