## Narrative

Employee had set the CM up in #6 entry.  EE was turning EE's body around to get better positioning, as EE turned EE tripped over a lump of coal and fell to the ground.  this resulted in a fracture to EE's right elbow.  EE has since had surgery to correct the fracture in EE's elbow.
