Narrative

Was shoveling longwall feet & was using left foot to brace self. Foot slipped causing to fall backwards on a piece of steel. Cause of injury wet conditions, body positioning. Incident didn't involve any aspect of compliance with rules/regulations/PPE. Miner proficiency may have been a factor. This is a Contractor for Dave Stanley Consulting. Started missing work 12/1/2016
