## Narrative

Employee was operating a roof bolter on the l Left Section.  EE had drilled 5ft of a 12ft hole and added a pusher to complete the hole.  EE started rotation and the kevlar sleeve became entangled on the drill chuck wrapping EE's arm around the drill steel resulting in a broken Humerus bone in left arm.
