## Narrative

Employee was performing daily maintenance on site boiler. The discharge auger was either plugged or not working. EE shut down blowers & opened boiler inspection door. EE hit the discharge trip lever to dump ash & that action caused hot ash to come up out of the door & hit EE in front torso & right side of face causing 1st degree burn to side of face & 2nd degree burn lip.
