Narrative

A fully trained miner was injured when loading supplies on a scoop. A roof bolt was under a skid of rock dust and when being winched onto scoop bucket the bolt sprung loose hitting the Rt. Knee. The injury was not a result of any compliance issues nor were any protective devices involved.
