Narrative

Employee was swinging a sledge hammer against the outside of a bulk storage bin to dislodge the material inside. The end of the hammer became entangled in a stainless steel lifeline cable, causing a strain to the employee's shoulder. The employee received first aid treatment initially, but was later placed on restricted duty by the medical provider on January 13, 2017.
