Narrative

Employee and co-worker were carrying a joint of 6"" pipe into a cross-cut when the employee struck head against a roof channel, causing pain in neck. The accident was caused by the employee's failure to pay attention to the area EE was walking in. Employee was wearing all required PPE but needs to improve attitude for safety.
