## Narrative

Miner walking from one location to another, uneven ground with loose rock. Miner slipped on loose rock causing miner to fall to the ground and land on right shoulder. Miner sought medical care on 11/18/16 and was released without restrictions. It was later determined that miner needed further medical care with restrictions beginning on 01/24/2017. Return to work full duty 01/30/17.
